Status epilepticus and encephalopathy caused by Shigella flexneri. Case report

It is recognized that shigellosis produces a wide range of intestinal and extra-intestinal complications, being seizures and encephalopathy frequent neurologic symptoms. These symptoms may appear prior to dysenteric diarrhea. An unusual case of shigellosis is reported of a two-year-old patient who presented with febrile status epilepticus and encephalopathy and who had Shigella flexneri in his stool. He presented no other metabolic or intestinal disturbances to explain his symptoms.
Conclusion: Shigella-induced encephalopathy has to be considered in the differential diagnosis of patients who present to the emergency room with diarrhea, and febrile seizures. Neurologic symptoms may appear before de onset of dysentery. A severe and potentially lethal form of shigellosis must be considered in patients with prolonged encephalopathy and status epilepticus.
