Background: Exclusive breastfeeding is the primary source of nutrition during the first six months of life. In Peru, breastfeeding rates declined after the pandemic. Self-efficacy is a modifiable factor in breastfeeding and can be assessed using the Breastfeeding Self-Efficacy Scale – Short Form (BSES-SF), originally developed in Canada. Objective: Obtain the cultural adaptation, validation, and reliability analysis of the BSES-SF in a Peruvian population. Methods: Descriptive cross-sectional study. The validity and reliability of the scale were evaluated in four phases: (1) linguistic adaptation to the local context, (2) content evaluation by an expert committee, (3) pilot testing in 30 postpartum women, and (4) analysis of content validity, internal consistency, and construct validity in a sample of 248 postpartum women. Results: The content validity reached a coefficient of 0.93, which shows an adequate representativeness of the items. The construct validity was confirmed by a confirmatory factor analysis, which supported a one-dimensional structure of the instrument. Likewise, Student’s t test (p < 0.001) showed significant differences between the groups evaluated. Reliability presented a Cronbach alpha of 0.774 and an intraclass correlation coefficient of 0.794, values indicating acceptable internal consistency and stability. Conclusion: The adapted version of the BSES-SF can be considered a valid and reliable measure of breastfeeding self-efficacy in Peru.
