Objective: To report a case of acute lymphoblastic leukemia (ALL) in a child with an initial diagnosis of polyarthritis. Materials and Methods: Review of clinical history and various diagnostic studies, including joint ultrasound, bone survey, bone scintigraphy, tomography, and bone marrow aspiration. Case Report: A 4-year-old boy from Iquitos, Peru, presented with migratory polyarthritis, joint pain, swelling, functional limitation, and intermittent fever, with persistent symptoms. Lytic lesions were identified in the right hand and radius, along with right axillary lymphadenopathy, hepatomegaly, anemia, leukopenia, and thrombocytosis. After unresponsive antibiotic treatment, a diagnosis of B-cell ALL was confirmed by bone marrow aspiration. The patient received chemotherapy, resulting in remission of joint symptoms. Conclusion: Articular manifestations as the debut of ALL, especially in the absence of peripheral blood blasts, pose a diagnostic challenge. Maintaining a high level of suspicion is crucial for early diagnosis and timely treatment, improving patient survival.
