Hipercyanotic espell

The espell hipercyanotic is a complication that patients with congenital heart disease present due to the anatomy which allows abrupt changes in the ratio of pulmonary to systemic blood flow. This complication is part of the natural history of Tetrallogy Fallot. An imbalance between pulmonary and systemic vascular resistance favoring decreased pulmonary flow and increased right –to – left shunting into the aorta produces an increasing hipoxemia and developing lactica acidosis. This will induce further pulmonary vasoconstriction. The known precipitating factors include: fever, anxiety, exercise, dehydratation, tachypnea, and tachycardia, some drugs that decrease systemic vascular resistance, such as chlorpromazine and ACE inhibitors. the immediate recognition of these symptoms and the convenient treatment prevent coma and death. Because of the importance of this pathology the following bibliographic revision is presented.
