Introduction: Retroperitoneal tumors are a group of rare neoplasms. Within them, the retroperitoneal teratoma represents approximately 4% of all the teratomas in the pediatric population. In Peru, there are currently no precise epidemiological records on its frequency or specific consensus guidelines for its management. Case report: We present a case from Trujillo - Perú referred to our hospital in Lima of a newborn with a left retroperitoneal tumor with laboratory tests were inconclusive. A percutaneous biopsy determined the case to be intermediate-risk neuroblastoma, and chemotherapy was initiated. However, the tumor exhibited rapid growth with marked abdominal distension and clinical deterioration, necessitating emergency surgical resection. Analysis of the specimen revealed an immature teratoma. After more than two years of postoperative follow-up, the patient showed no recurrence or relapse. Conclusion:The biopsy is a fundamental diagnostic tool in the evaluation of retroperitoneal tumors; however, other non-invasive techniques can aid in diagnosis, including exhaustive radiological analysis of images. A multidisciplinary approach is essential for defining timely diagnostic and therapeutic strategies for the benefit of the patient.
