We present the case of a preterm newborn who presented with respiratory distress at five minutes after birth. A chest x-ray was requested, suggesting probable congenital pneumonia and antibiotic treatment was initiated. A contrast-enhanced CT scan was subsequently performed, concluding that a congenital lung malformation of the intralobar pulmonary sequestration type was present. A teleconsultation was conducted with the thoracic and cardiovascular surgery department (Instituto Nacional de Salud del Niño – Lima), who recommended discharge for subsequent surgery starting at 3 years of age. This case is interesting because it requires us to perform a differential diagnosis with all possible causes of congenital lung malformations in the newborn, not only considering the most common, but rather adopting a rational approach and timely referral.
