We present the case of a newborn male diagnosed with transient neonatal hyperthyroidism, a rare condition in clinical practice. The main clinical manifestation during the adaptation period was persistent hypoglycemia, accompanied by signs of hypercatabolism. The patient's mother had a history of hyperthyroidism diagnosed in 2019 with irregular treatment with antithyroid drugs. The newborn was diagnosed at 8 days of age, considering maternal history and clinical signs such as low birth weight, small for gestational age, tremors, poor suckling, persistent hypoglycemia, and poor weight gain. Thyroid profiles confirmed the suspicion: TSH 0.121 uUI/ml, free T4 41.58 pmol/l, TSI 104%. Treatment was initiated with thiamazole and propranolol, with favorable clinical outcome after therapy. This case represents an unusual presentation of transient neonatal hyperthyroidism, given that its main manifestation was hypoglycemia, a rare finding in this entity.
