Abstract

Glomerulopathies in children frequently present with persistent hematuria or proteinuria, which requires confirmation by renal biopsy. IgM nephropathy (IgMN) is a rare glomerular disease characterized by IgM deposits in the mesangium. We present the case of a 2 years old patient with persistent gross hematuria, diagnosed with IgMN after a renal biopsy, who showed resistance to initial steroid treatment. Justification of the clinical case: This case is presented due to the rarity of IgM nephropathy in pediatric patients whose initial manifestation is persistent gross hematuria, in addition to the finding of diffuse podocytopathy without visible mesangial deposits on electron microscopy. This combination of findings represents a rare diagnostic and therapeutic challenge, which justifies its detailed reporting and analysis to contribute to the clinical and pathological understanding of this entity. Early management of IgM nephropathy is crucial to prevent progression to chronic kidney disease, highlighting the need for timely diagnosis and close monitoring in pediatric patients.
