Introduction. There can be benign anisocoria; however, it is important to recognize the main causes that produce it, such as traumatic due to compression of third cranial nerve, Adie’s pupil, Horner’s syndrome, drugs, physiological, vascular, toxicological, brain structures herniation.
Case report. One year-old male infant diagnosed with atypical pneumonia, in managed with micro nebulized bronchodilators.
Showing up with sudden left anisocoria at 3 days of treatment started with ipratropium bromide, without any more findings at physical nor neurological examination. Simple TC showing frontal cortical atrophy without any mass effect or bleeding signs. Discarding a central neurological problem, a pharmacologically associated unilateral mydriasis diagnosis was given. It was handled by suspending ipratropium bromide micro nebulizations, showing resolution at 14 hours.
Comment. Pharmacological anisocoria on infants due to nebulized medications is a diagnosis the pediatrician must have in mind.
