Hypokalemic periodic flaccid paralysis is a very rare, reversible, episodic neuromuscular disorder related to a defect in muscle ion channels associated with transient changes in potassium concentration. It is important to know how to identify, to know the differential diagnosis and its possible complications. We present the case of a 12-year-old man who entered the emergency service of the Moquegua Regional Hospital with loss of muscle strength in the upper and lower limbs, accompanied by respiratory distress, serum potassium <2mmol / L, alteration of the electrocardiogram: long QT, U wave. After the potassium challenge, there is clinical and laboratory improvement. Early diagnosis allows timely treatment, avoiding complications.
