The term M. tuberculosis is commonly used as a synonym for any of the mycobacteria that form part of the "Mycobacterium tuberculosis Complex". However, there are important differential characteristics in their epidemiology and management.
We describe the case of a 3-month-old girl without precedent of TB contact, with pulmonary, mediastinal and immunological compromise, whose biopsies showed a chronic granulomatous inflammatory process, BK (+) and fast cultures, in liquid media, with molecular genetic study, allowed to identify the causal agent: Mycobacterium canettii, sensitive to tuberculostatics of 1st and 2nd line.
